Vaccine Clinics for Anyone Age 5 and Up

Hello Park Side families,
 
I trust you all had a wonderful Thanksgiving week.
 
The second round of COVID Vaccine Clinics for ages 5 and up is scheduled for 12/08/21 and 12/09/21 at Brook Haven School. There are clinics in Occidental and Guerneville, as well. If your child missed the first vaccine clinic options, they will be able to give some first-time vaccines to those that need them. Please just come to one of the school-based clinics during open hours and they will try to get you in. If you are not able to bring your child, children must provide a signed consent form.

The upcoming clinics (see flier) are:

  • Weds. 12/1 at Salmon Creek/Harmony School in Occidental 3pm-6:30pm
  • Weds. 12/8 at Brook Haven School  2:30 pm-6:30 pm &
  • Thurs. 12/9 at Brook Haven School 1:30 pm.-5:30 pm
Please note: If you had scheduled an appointment for the 1st dose (in November) the appointment time for the above clinics was presented for selection at that time. Walk-ins will be accepted and as Ms. Irving notes above, please just come during open hours and they will try to get you in. 
 
First Dose Appointments:
You can make an appointment to get the vaccine by clicking here: https://myturn.ca.gov/. Appointments can only be made within one week before the scheduled clinic date. Making an appointment is highly recommended to reduce your wait time, but walk-ins will be accepted.
 
Minor consent forms for any minor under 18 years of age are required. Please print the below minor consent form and have your child bring it with him/her to school. A parent/guardian does not need to be present, but we do need the signed consent form in order to administer the vaccine. Please note, students will NOT be vaccinated without consent!
